AN IN-HOME CONSULTATION
First, we visit with the older adult and his/her family members as appropriate at home. This complimentary visit gives us the opportunity to get to know the older adult better and begin to put together the services that would best fit in his/her her particular situation.
A CUSTOMIZED CARE PLAN
Our registered nurse will the create a plan of care and personalize it for the older adult. Each care plan outlines the services we recommend including when in-home services will occur based on the desired routine and schedule. The care plan will be regularly assessed and adjusted as needed.
